Subject: In accordance with rules and procedures implementing the State Drinking Water Revolving Loan Fund, an environmental assessment (EA) has been performed by the Oregon Department of Human Services – Drinking Water Program (Department) on the proposed project water system upgrades, Christmas Valley, Oregon.

Purpose/Summary: This project will replace an existing wood storage tank with a steel storage tank and replace approximately 5 1/2 miles distribution water piping.

Finding: The Department has reviewed the environmental information document (EID) submitted by Christmas Valley Domestic Water Supply District. Following the review, the Department has issued an environmental assessment (EA) determining that an environmental impact report (EIR) is not necessary. The Department hereby issues a Finding of No Significant Impact (FNSI). This decision is based on a review of the information contained in the EID and any supporting data. HOSPITAL PHASE I SITE CONSTRUCTION

Separate sealed bids for the LAKE DISTRICT HOSPITAL PHASE I SITE CONSTRUCTION located in Lakeview, Oregon, will be received by Gordon Ensley, Lake Health District, 700 South “J” Street, Lakeview, Oregon 97630, until 2:00 P.M., local time, September 16, 2008. The bids will be publicly opened, announced, and made available for public inspection at 2:00 PM on September 16, 2008. The First-Tier Subcontractor Disclosure Form, which is submitted separately, is required at 4:00 PM on September 16, 2008. The bids shall be mailed or delivered; fax transmissions will not be accepted.

Generally the work consists of demolition of asphalt and concrete areas, construction of three parking lots, installation of a 10” water line, construction of a heliport, construction of a service road, and other related site work.

CONTRACT DOCUMENTS may be obtained from Anderson Engineering and Surveying, Inc., P.O. Box 28, 17681 Hwy 395, Lakeview, OR 97630, for the non-refundable pre-paid cost of $200.00 per set.

Bidders must be pre-qualified. Bidders must submit by mail or deliver the Contractor’s Pre-Qualification Form, which is included in the contract documents, or the State of Oregon Department of Administrative Services Pre-Qualification Application Form at least seven (7) working days prior to bid opening. Fax transmissions of Pre-Qualification Forms will not be accepted. Bidders will be notified of approval or rejection of their pre-qualifications within two days after receipt of the Pre-Qualifications.

This is a public works contract and no bid will be received or considered by the public contracting agency unless the bid contains a statement by the bidder, as part of their/his/her bid, that the provisions of ORS 279.350 (Prevailing Wage Rates) are to be complied with. A copy of the Prevailing Wage Rate Statement is included in the contract documents.

No bids will be considered unless fully completed in the manner provided upon the bid form provided by the owner and accompanied by the bid bond or a certified check executed in favor of the owner in an amount not less than 5% of the total amount of the bid. Bid bond or check will be forfeited should the selected bidder neglect or refuse to enter into contract and provide suitable bond for faithful performance of the work.

The successful bidder shall provide a performance bond, as hereinafter stipulated, for the full amount of the contract within 10 days after awarding of the contract, i.e. contract date.

No bidder may withdraw or modify their bid after the hour set for the opening thereof until after the lapse of thirty (30) days from the bid opening. The owner reserves the right to reject any or all bids not in compliance with all prescribed bidding procedures and requirements and other applicable laws, and the owner may reject for good cause any or all bids upon the owner finding that it is in the public interest to do so. Owner may waive any informality in the bidding process.

There will be a MANDATORY Pre-Bid Meeting September 9, 2008 at the 2:00 PM in the Lake District Hospital Conference Room, 700 South “J” Street, Lakeview, Oregon 97630

Bidders must be registered with the Oregon Construction Contractors Board.
